What does this mean ? It means that in conceptual photography the actual picture comes secondary. First there is an idea or call it concept that you have created in your mind, usually based on a feeling or experience, for example sadness, or a death in your family. You want to demonstrate and express this with a photograph, to share your experience or feeling and portray it, so the viewer can feel the same by looking on your picture. Once you have achieved that the viewer feels the same you do, then you have created a conceptual photograph. Creating such a piece of Art does not necessarily require to have a person as a model, in fact most of the times portraits are just portraits with little conceptual ideas behind them, most of the times if done right, using real objects can be used to achieve a better effect. It aways helps to add a description to your art so the viewer can understand why you have portrayed your concept in this specific way.
Keep in mind, taking a random shot and dreaming an idea into it, is NOT what conceptual art is about ! First comes the idea / concept that you created in your mind and the actual picture comes secondary.

*edit: the above fits more into Conceptual Studio work, of course there is other conceptual work where you need to be "fast thinking" especially when it comes to Conceptual work in Nature or Street photography ( just as an example) more about this at a later point.
